Pathology

A testis with a regressed germ cell tumor usually has one or more ill-defined areas of parenchymal scarring (Fig. 7.27). The scars are composed of dense fibrous tissue, usually with infiltrates of lymphocytes and scattered hemosiderin-laden or lipid-laden macrophages, and sometimes with areas of dystrophic calcification (Fig. 7.28). Occasionally, residual ICGN or foci of viable germ cell tumor may be present.

Fig. 7.27Regressed (“burned-out”) germ cell tumor. A retroperitoneal mass in this 37-year-old man proved to be seminoma on biopsy. Although no definite testicular mass was palpable, ultrasound disclosed this lesion, prompting orchiectomy (From MacLennan et al. (2003), with permission)
